I was trying to keep the report short.  As others have commented:

Commercial file formats have multiple versions, seemingly every year.
PowerPoint, Word,
Excel, Flash, Portable Document Format, Java and class files,
ECMAscript, html, 3D software, etc.

Then there are the container formats that can include an open-ended
set of other formats: TIFF, Quicktime, and MIDI. Plus XML with the
CDATA construct.
